It operates through two segments: home health and hospice services and senior living services. The home health and hospice services segment include its home health, hospice and home care businesses. The senior living services segment includes the operation of assisted living, independent living and memory care communities. Its senior living operations provide a variety of services tailored to its residents’ needs, including residential accommodations, activities, meals, housekeeping and assistance in the activities of daily living to seniors. It provides healthcare services through 122 home health and hospice agencies and 56 senior living communities located throughout Arizona, California, Colorado, Connecticut, Idaho, Montana, Nevada, Oklahoma, Oregon, Texas, Utah, Washington, Wisconsin and Wyoming.
